
Many of us spend most of the day sitting:
At a desk, in the car, during meals, and in front of a screen. In high-income countries, adults may be sedentary for approximately 9 to 11 hours each day, which is more than half of their waking hours.
That matters because the human body was designed to move regularly, not simply to complete one workout and then remain inactive for the rest of the day.
Research consistently links high amounts of sitting, often defined as more than eight hours per day, with an increased risk of type 2 diabetes, cardiovascular disease, some cancers, and premature death. The association is particularly strong for type 2 diabetes, where excessive sitting time almost doubles the risk.
Sitting also increases the incidence and mortality risk associated with other common chronic diseases, including cancer and cardiovascular disease, by approximately 10% to 20%.
The encouraging news is that you do not need to overhaul your schedule to begin making a difference. A five-minute walking break every hour, or more frequently when practical, can reactivate your muscles, improve circulation, and help you feel more energized.
Is sitting "the new smoking"?
You may have heard the phrase "sitting is the new smoking." It is a memorable comparison, but scientifically, it is an exaggeration.
Smoking is substantially more dangerous than prolonged sitting. One analysis estimated that sitting was associated with an approximately 22% higher risk of premature death, while current smoking was associated with an approximately 180% higher risk compared with never smoking. Smoking also directly exposes the body to toxic and carcinogenic substances in a way that sitting does not.
Sitting is not the new smoking.
That said, prolonged sitting is not harmless. It occupies an enormous portion of modern life, and its effects can accumulate over years. Excessive, uninterrupted sitting is therefore worth treating as a meaningful and modifiable health risk.Why is prolonged sitting bad for the body?
Prolonged sitting can affect health through two closely connected mechanisms: the large muscles of the legs become almost completely inactive, and the seated position can reduce and disturb blood flow through the lower limbs.
Your muscles are metabolic organs
Muscles do much more than move your body. Every time they contract, they use energy and send biochemical signals that help regulate metabolism throughout the body.
When you stand or walk, the muscles in your calves, thighs, and hips repeatedly contract. These contractions help your body use glucose from the bloodstream, process circulating fats, support insulin sensitivity, maintain blood flow, and activate cellular energy systems.
When you sit, the large muscles of the legs require very little energy. Their uptake and processing of glucose and fats decline, and some of the beneficial signals produced by regular muscle contraction become less active.
Over many hours, repeatedly remaining in this low-activity state may contribute to poorer regulation of blood sugar and blood fats. Active muscles behave like metabolic engines, and prolonged sitting leaves those engines idling.
Sitting also changes blood flow in the legs
When you sit, your hips and knees remain bent, which creates multiple bends in the arteries of the lower body. At the same time, the calf and thigh muscles are no longer contracting frequently enough to assist circulation.
The resulting reduction in blood flow changes the mechanical forces experienced by the inner lining of the arteries. Healthy blood flow normally produces a force called shear stress, which helps blood vessels remain flexible and responsive. When blood flow and shear stress decline, vascular function may temporarily worsen.
A systematic review found that lower-limb vascular function became progressively impaired after approximately two to three hours of uninterrupted sitting. The same pattern was not clearly observed in the arms, which supports the idea that both leg-muscle inactivity and the seated position contribute to the effect. Fortunately, regularly interrupting sitting with walking or other active movement can help preserve circulation.
"Sitting too much" and "exercising too little" are not identical
A person can exercise regularly and still spend most of the day sitting. Someone might complete a 45-minute workout in the morning and then sit for ten hours while working, commuting, eating, and relaxing. That person meets standard exercise guidelines but remains highly sedentary for the majority of the day. Structured exercise and movement throughout the day provide overlapping but distinct benefits. Exercise builds cardiovascular fitness, strength, endurance, and long-term health. Movement breaks reactivate muscles and circulation during hours that would otherwise be sedentary.
A morning workout does not make movement breaks unnecessary, and movement breaks do not replace structured exercise. The strongest approach is to include both.
What happens when you interrupt sitting?
Researchers have tested many different movement patterns, including five minutes of walking every hour, a few minutes of movement every 20 to 30 minutes, 90 seconds of activity every 15 minutes, and longer activity breaks every one to two hours. Although the exact protocols vary, the overall conclusion is consistent: interrupting prolonged sitting with physical activity can improve the way the body handles glucose, insulin, and fats after meals.
A systematic review and meta-analysis led by Roland Loh examined 37 studies of physical activity breaks and found that interrupting sitting moderately improved post-meal glucose, insulin, and triglyceride responses. The improvements in glucose regulation tended to be greater among people with a higher body mass index. When researchers matched the total energy expended, spreading activity into short breaks throughout the day produced a small advantage for glucose control compared with completing the same activity in one continuous session. The difference was not significant for insulin or triglycerides.
This does not mean that a five-minute walk is better than a structured gym session. It means that the timing of movement matters, because muscles benefit from being reactivated repeatedly throughout the day.
The Body Electric Study: Could five-minute breaks work in real life?
Many early movement-break studies were conducted in controlled laboratory settings with relatively small participant groups. Researchers wanted to know whether people could realistically adopt this behavior during everyday life.
The Body Electric Study, developed by researchers at Columbia University, evaluated a two-week movement challenge promoted through the Body Electric podcast. Participants selected one of three schedules: a five-minute movement break every 30 minutes, every 60 minutes, or every 120 minutes. Each participant maintained the same schedule throughout the study. The self-selected design reflected the reality that people differ in their work, health, responsibilities, and ability to step away from their desks. More than 11,000 adults contributed data to the evaluation, making it far larger than a conventional laboratory study.
All three schedules were considered generally feasible and acceptable, although taking a break every 30 minutes was more demanding than the others. Notably, the movement breaks did not appear to reduce work productivity. Participants reported improvements in fatigue, mood, work engagement, and performance, with the most frequent breaks generally producing the largest reduction in fatigue. The study was short, observational, and based on self-reported outcomes, so it cannot establish long-term disease prevention. Nevertheless, it provides valuable evidence that brief walking breaks can be integrated into real working life, and suggests that doing so may help people feel better throughout the day.
Why might a short walk improve energy?
It may seem counterintuitive that expending energy on a walk could make you feel less tired. But fatigue during a sedentary day is not necessarily caused by a lack of available calories. A short walk can increase blood flow throughout the body, reactivate the large muscles of the legs, increase oxygen delivery, provide a mental reset, interrupt monotonous or stressful work, and support alertness and mood. Movement may therefore help you feel more energized even though you are using slightly more physical energy.
Can exercise offset the risks of sitting?
Higher levels of physical activity can substantially reduce the long-term risks associated with prolonged sitting. A major analysis involving data from more than one million adults found that approximately 60 to 75 minutes of moderate-intensity physical activity per day appeared to eliminate the increased mortality risk associated with sitting for more than eight hours.
That is reassuring, but 60 to 75 minutes of daily activity is considerably more than most people currently achieve.
Around 30 minutes of daily physical activity also appears to reduce the risk associated with very high sitting time, though it may not eliminate it entirely. Short movement breaks can contribute toward that daily total while also preventing long periods of complete muscular inactivity. The practical takeaway is not to choose between a structured workout and movement breaks throughout the day. Both matter. Regular aerobic and strength-building exercise remains important, and breaking up long periods of sitting adds meaningful benefit on top of it.Why walking breaks may be more practical than specialized equipment
Consider this:
Standing desks, treadmill desks, and under-desk bicycles can be helpful for some people. However, they can also be expensive, require space, and may not fit every workplace.
Simply standing still may not provide the same benefits as walking because standing produces relatively little muscle activity. Research on whether replacing sitting with stationary standing meaningfully improves metabolic health remains mixed.
Walking, by contrast, is familiar, free, and accessible to many people. It activates the calf, thigh, and hip muscles while increasing circulation without requiring special equipment. Even a trip to refill your water bottle counts.
Try the five-minute movement snack
A realistic starting target is to walk or move for five minutes after every 60 minutes of sitting. During your movement snack, you could walk around your home or workplace, take a phone call on the move, climb a flight of stairs, march in place, perform gentle body-weight squats or calf raises, step outside for a brief mental reset, or complete a small household task.
Set a recurring reminder on your phone, watch, or computer until the behavior becomes more automatic. Walking every 30 minutes may produce greater short-term benefits, particularly for fatigue, but an hourly break may be easier to maintain consistently. The best schedule is simply the one you can stick to.
Let's Get to The Standing Point
Sitting is not as dangerous as smoking, but spending most of the day seated and motionless is not biologically neutral. When your leg muscles remain inactive, they use less glucose and fat and produce fewer of the signals that support healthy metabolism. At the same time, prolonged sitting reduces blood flow and can temporarily impair vascular function in the legs. Regular exercise remains essential. Your body also benefits from frequent reminders that it was designed to move.
A five-minute walk may seem small. Repeated throughout the day, however, it can reduce fatigue, reactivate your metabolic engines, support circulation, and contribute to better long-term health. The goal is not only to exercise more. It is to sit less and to break up the sitting you cannot avoid.
